As part of our onboarding process, all clinicians are required to complete this Age-Specific Performance Expectations Evaluation to demonstrate their understanding of the unique needs of different patient populations.
Purpose of the Evaluation
This evaluation is designed to assess your knowledge and competency in delivering care to patients across different age groups. Each age group presents unique physical, emotional, and developmental needs that must be understood to provide appropriate care.

Infants (0-12 months)
- Proper handling and positioning techniques
- Recognizing signs of distress in non-verbal patients
- Safe feeding and nutrition practices
Toddlers (1-3 years)
- Managing separation anxiety
- Safety measures and fall prevention
- Effective communication techniques
Children (4-12 years)
- Age-appropriate communication
- Encouraging cooperation during procedures
- Recognizing developmental milestones
Adolescents (13-17 years)
- Respecting privacy and autonomy
- Addressing mental health and emotional needs
- Providing education on health-related topics
Adults (18-64 years)
- Patient-centered communication
- Chronic disease management
- Encouraging self-care and preventive measures
Geriatric Patients (65+ years)
- Fall prevention and mobility support
- Cognitive assessment and dementia care
- Medication management and polypharmacy awareness

By completing this evaluation, you agree to:

- Continuously update your knowledge and skills related to age-specific care
- Report any concerns or gaps in competency to your supervisor

All evaluation results are confidential and will only be shared with authorized personnel for the purpose of verifying your competency and eligibility for assignments.
